Recreation Item Checkout Form

Items from the Atlantic Parks and Recreation Department can be checked out free of charge. Once you have submitted the form with what items you are wanting a follow up email will let you know if those items are available that day. Items can be picked up at the Parks and Rec Office during office ours. If you are picking them up after office ours please call the Parks and Rec Office to set up a time. I, the above signed, hereby agree that I am requesting permission from Atlantic Parks and Recreation of the City of Atlantic, Iowa the temporary use of the above marked equipment. In barrowing and using the above marked equipment for activities, I understand that there exists a risk for personal injury as a result of my personal and voluntary interaction with the equipment. On behalf of myself, my heirs, personal representatives, and executor, I hereby release, discharge, indemnify, and hold harmless the City of Atlantic, Iowa, and The Atlantic Parks and Recreation, it's agents, servants, and employees from any and all claims, causes if action, or demands of any nature or cause connected with my personal and voluntary use of the above marked equipment. This could include any cost, attorney's fees, and court costs incurred by the City of Atlantic, Iowa, in connection with my personal use of City property based on aforementioned activities, Such damages may include, but are not limited to accidents, injures, and personal property damage. I understand that any damage to barrowed City property may be my responsibility to financially cover the replacement of or repair of the damaged items. Underage use of equipment: As the parent or legal guardian I take full legal responsibility and give my full consent to allow my child/children, or ward/wards, to use the equipment barrowed on loan from Atlantic Parks and Recreation. I have read and fully understand the terms and conditions of this waver of liability agreement and I willingly comply with all of its conditions, on behalf of myself, my child/children, ward/wards, and any other active participant, I agree to all of the terms and conditions outlined in this waver of liability agreement.
